Welcome to the Evolent provider portal registration wizard.

If you are already an Evolent provider portal user under your current email address, please do not complete the online self-registration. If you are locked out of your account, forgot your account username, or you need assistance updating your associated provider(s) or practice(s), please contact Evolent Provider Solutions at providertraining@evolent.com or 1.888.999.7713, option 6. If you do not have an existing Evolent provider portal profile, please proceed with the online registration.

Portal registration must be requested under the physician organization (professional) tax identification number (TIN) of the ordering provider. Please note that if you are with a servicing/rendering facility that does not employ the ordering physicians, Evolent will request consent from the ordering physician prior to granting access to the Evolent portal for that respective TIN.

Once you have completed the online registration, you will receive an email with your account username and temporary password. This will enable you to:

• Submit prior authorization requests
• Track and manage authorizations you've submitted via the provider portal
• Verify member eligibility
• Access valuable resources in Useful Tools > Useful Documents

1. To ensure compliance with applicable federal and state laws, rules and regulations, and with health plan requirements ("Applicable Requirements"), your practice must attest that its use and access complies with all Applicable Requirements prior to accessing the Evolent system. Registered users are responsible for, and retain all liability associated with, any access to the system outside of the United States without permission. Failure to comply with this requirement will result in termination of access and other disciplinary actions.
